Population Health Management Recorded Webinar

Leveraging Payment Models to Achieve Population Health Management Success: Strategies for Real-World Action

Please join Population Health Management (PHM) leaders from the ACR for a webinar on how to leverage shared risk and alternative payment models to deliver high-quality care to distinct patient populations. In the years ahead, PHM will provide a blueprint for radiology’s role in keeping disease screening front-and-center, optimizing appropriate care, and helping guide chronic disease management. Join us to better understand how the value of high-quality radiologic care can be recognized as a key component in improving the overall health of populations, and how alternative payment models — both public and private — can help incentivize PHM adoption and lead to success for radiologists.

Learning Objectives

  • Identify the different risk-sharing APMs (both CMS and private payer models) as they relate to PHM in practice settings ranging from private to academic to community practice.
  • Recognize how clinical decision support, lexicons, risk stratification systems, and other strategies within health systems can be utilized to manage appropriate imaging of patient populations, and to help design quality measures around such efforts.
  • Describe screening methodologies that can be used for community-based PHM.
  • Explain the use of appropriate recommendations for clinical diagnoses and enrolling patients into follow-up programs.
