The ACR National Radiology Data Registry (NRDR®) is a CMS-approved QCDR for the Merit-Based Incentive Payment System (MIPS) for 2025. In 2025, the ACR® NRDR QCDR includes five new measures in addition to the QCDR measures available in 2024:

  • ACRad 43: DXA — Improving Reporting of True Change in Bone Mineral Density
  • QMM 23: Low-Dose Cancer Screening Recommendation for CT of Chest With Diagnosis of Emphysema
  • QMM 24: Acute Rib Fracture Numbering on ED Trauma Patients
  • QMM 27: Appropriate Classification and Follow-up Imaging for Incidental Pancreatic Cysts
  • QMM 28: Reporting Breast Arterial Calcification (BAC) on Screening Mammography

View the full 2025 QCDR measure specifications. Practices can begin reporting 2025 MIPS data to the NRDR QCDR at any time.

MIPS Scoring Updates

The 2025 MIPS program also brings some scoring changes, which are likely to positively affect radiologists. In the 2025 Medicare Physician Fee Schedule Final Rule, CMS indicated that certain “topped out” quality measures that belong to measure sets in which most measures are topped out will now be eligible for 10 achievement points.

Several diagnostic radiology measures that have been capped at seven points in recent years will now be eligible for 10 points in 2025. The performance threshold remains at 75 points, and the quality measure data completeness requirement remains 75%.

As a reminder, physicians participating in 2024 MIPS must complete CMS submission by March 31 at 8pm ET.
