The ACR RLI Leadership Essentials course equips trainees with the professional skills needed to navigate an increasingly complex healthcare environment.
Radiology trainees occupy a unique stage in their careers. Their primary goal is to master diagnostic and procedural skills while also investing hours in studying, research and institutional responsibilities. At the same time, they are preparing for fellowship or practice and entering a profession that continues to evolve rapidly. One area that has historically been underemphasized in radiology training is preparation for the practical, professional and leadership responsibilities that come with a career as a practicing radiologist. As radiology continues to evolve in response to the clinical applications of AI, changing reimbursement models, multidisciplinary care and increasing administrative complexity, providing trainees the opportunity to develop these skills is something radiology training programs should strongly consider.
The ACR Radiology Leadership Institute® (RLI) Leadership Essentials program is designed to help address this gap. The longitudinal program provides residents and fellows with a comprehensive introduction to the practical, professional and leadership skills needed to navigate their careers, work effectively with others and contribute meaningfully within a practice or healthcare organization.
The program includes four months of flexible virtual learning through eight on-demand lectures and monthly live question and answer sessions with expert faculty. Topics include personal finance, negotiation, effective communication, AI and making yourself indispensable. Some of the curriculum encourages collaboration among the residents and some are intended for individual reflection and application.
There are some aspects of practicing radiology that trainees cannot learn in the reading room. Leadership Essentials pushed me to start thinking about networking, mentorship and marketability early on in my career.
Leadership Essentials is designed to be flexible and can be easily incorporated into residency programs in different ways to fit each program’s structure and needs. We have first-hand experience with the Leadership Essentials program as our first-year residents (R1s) and chief residents participated in it over the last academic year. Because of the flexibility of the program, we were able to supplement the core curriculum with additional components, including a capstone project and personality leadership assessment. We felt these were meaningful additions that helped participants apply and reinforce concepts they learned from the course.

More widespread implementation of the course would be beneficial to first year radiology residents in all programs.”
We chose to enroll the R1s so they could benefit and apply these skills throughout the remainder of their residency. In addition, the clinical and academic demands of the R1 year are often lighter than those of the R2 and R3 years when residents face high call expectations and ABR CORE exam preparation. Early exposure to these concepts can profoundly influence not only a resident’s career trajectory but also the culture of the residency program and ultimately the specialty.
At the same time there is also a strong argument for offering the program during the R4 year as residents prepare for fellowship or practice. It would also be particularly valuable for chief residents or programs with an established leadership track.
The Leadership Essentials program is a valuable complement to traditional radiology education because it prepares residents for many of the real-world responsibilities that are not historically addressed in their residency training. While residency programs appropriately emphasize the development of clinical knowledge, there is often limited formal instruction for topics such as negotiation, effective communication, AI applications, healthcare economics, etc.
As radiology continues to evolve, preparing residents for successful careers requires more than clinical excellence alone. Programs such as Leadership Essentials help trainees develop the professional skills needed to navigate an increasingly complex healthcare environment, contribute effectively within their organizations and confidently transition into practice. Encouraging participation in Leadership Essentials as well as other RLI programs, can help ensure the next generation of radiologists is equipped not only to deliver outstanding patient care, but also build fulfilling, impactful and sustainable careers.
