CMS finalized the Excessive Radiation Dose or Inadequate Image Quality for Diagnostic CT in Adults electronic clinical quality measure (eCQM) in 2024, to encourage safer, more consistent CT imaging practices for voluntary reporting in 2025. The agency’s intent was to make the measure mandatory by 2027, for use in the Hospital Outpatient Quality Reporting Program, but after receiving strong public feedback the agency now proposes to indefinitely keep the eCQM voluntary. CMS cites its reversal on significant resource and infrastructure challenges, especially for facilities lacking the necessary technology.

ACR® supports keeping the measure voluntary. While the College strongly advocates for radiation safety and appropriate imaging through initiatives including Image Wisely, Image Gently and the Dose Index Registry, it recognizes the burdens the measure places on imaging facilities and hospitals.

Call to Action

ACR urges members to submit comments to CMS today to keep the Excessive Radiation Dose eCQM indefinitely voluntary. Comments must be submitted by Sept. 15 .
