Chapter Recognition Awards

The ACR Chapter Recognition Program was created in 2003 with the following goals:

  • To formally recognize chapter work, successes, and innovative ideas
  • To facilitate the sharing of ideas among chapters, thus encouraging and supporting the activity of all chapters

Chapters submit award forms for the activities or initiatives they engage in between January and December of the award year. Submissions are made in four categories: Government Relations, Meetings & Education, Membership, and Quality & Safety. The forms for the 2023 award cycle are now closed.

In addition, the Overall Excellence Award honors chapters who have demonstrated excellence in every award category.

Chapter Recognition Form Requirements

Please report your chapter activities for the award year using the forms below. Chapters may submit forms in multiple categories, or just one. Each category has a required form that must be submitted in order to be considered for an award. The Chapter Recognition: Share a Successful Practice form is optional. To earn additional points, a TOTAL of 5 Share a Successful Practice forms may be submitted across multiple categories or all 5 in a single category. The forms for the 2023 award cycle are due January 15, 2024.

Award Forms in Microsoft Word

Word versions of the forms are provided as a convenience, so that Chapters may work on them offline. However, Word forms will not be accepted as final submissions. All data must be transferred to the online forms. 

Government Relations Award

State- and federal-level government relations activities are eligible for recognition points. Please report your chapter’s activities, initiatives and successes using the appropriate form. Meetings & Education Award

The College recognizes chapters for meeting the education and networking needs of the membership. Please report your chapter’s activities, goals and successes using the appropriate forms. Membership Award

The College rewards chapter efforts in providing services and opportunities for members in various stages and specialties of the profession. Please report your chapter’s activities, goals and successes using the appropriate forms. Quality & Safety Award

Awards recognize chapters that document a continued commitment to promote and inform on quality- and safety-related matters. Please report your chapter’s activities, goals and successes using the appropriate forms. Overall Excellence Award

The Overall Excellence Award honors chapters who have verified excellence across all categories. All of the required forms must be submitted to be considered for Overall Excellence.

When you submit required forms in the Membership, Meetings & Education, Government Relations and Quality & Safety categories, then you are automatically considered for the Overall Category. You do not need to submit them twice. *Recommended browser for Chapter Recognition Forms: Google Chrome
