Apply for Accreditation
If your facility is applying for theStereotactic Breast Biopsy Accreditation Program for the first time, please click here to apply online.
If your facility is NOT applying for the Stereotactic Breast Biopsy Accreditation Program for the first time, please follow the instructions below:
Step 1 - Print and Review the Application Instructions and Checklist
- Application Instructions: Use the instructions as a guide as you complete the application forms. This document also provides definitions for frequently used terms
- Application Checklist: Use the checklist to ensure that all required items are included.
Step 2 - Complete the Required Forms
- Practice Site Forms: Please complete one set of forms for each practice site location.
- Legal Form: This legal form (one per practice site location) is required and must be submitted with the completed application form and payment.
- Modality Forms: Please complete the modality-specific equipment page for each unit at each site location. Please reference the Application Instructions for information that will assist you with completing these forms.
Step 3 – Submit the Application to the ACR

Testing and QC Forms
Facilities that have applied for the Stereotactic Breast Biopsy Accreditation Program will submit the following documents with their image submission for review. These forms are provided to the facility in hard-copy form once their initial application has been processed at the ACR. For your convenience, additional copies may be printed by clicking on the links below. Please note that these forms should not be submitted with an initial application for accreditation.
