Testing and QC Forms
Facilities that have applied for the Nuclear Medicine/PET Accreditation Program will read the following documents and use the forms to gather data that will be entered into the online testing package when the images are sent for review. Please note that these forms should not be submitted to the ACR for accreditation.
IMPORTANT - The Nuclear Medicine and PET accreditation process has recently changed.
NOT USING THE CORRECT FORMS MAY RESULT IN A DELAY OF THE ACCREDITATION PROCESS!
If you applied for nuclear medicine or PET accreditation:
- AFTER 2/22/10, use the links below.
- PRIOR to 2/22/10, please contact the ACR at 800-770-0145 and select option 6 to obtain the correct paperwork.
