Personnel, Testing and QC Forms
The ACR sends the following documents with testing materials to the facility after the initial application has been processed. The facility will usually submit the completed hard-copies at the same time they submit their images for review. For your convenience, additional copies may be printed by clicking on the links below. If you have any questions about which forms apply to your facility, please refer to the checklist and instructions in your accreditation testing package.

- The Direct Reporting of Results to Women by Mammography Facilities
- Sample Lay Report for Negative or Benign Findings (to be used with BI-RADS® 1-2)
- Sample Lay Report for Negative or Benign Finding(s) and Patient has Physical Findings, Signs or Symptoms (to be used with BI-RADS® 1-2)
- Sample Lay Report for Probably Benign Finding, but Initial 6-Month Follow-up Recommended (to be used with BI-RADS® 3)
- Sample Lay Report for Abnormal Fiding (to be used with BI-RADS® 4 and 5)
- Sample Lay Check List Report to be Handed to Patient if On-Line, Same Day Reading Is Done
