Attention

Due to feedback from our applicant facilities, the ACR has simplified the application process. Facilities applying for the Diagnostic Modality Accreditation Program (DMAP) will no longer be required to submit attestation forms for their personnel. In addition, automatic status updates will be e-mailed to facilities throughout the accreditation process. Beginning mid-October 2006, existing DMAP sites that wish to add a modality and new facilities will be able to apply using the ACR's new online accreditation application.

The Diagnostic Modality Accreditation Program incorporates all the ACR accreditation programs (except mammography and radiation oncology) under one application process. Below are 3 steps that will guide you through the application process.

 

Step 1

We recommend that you carefully read the Diagnostic Modality Accreditation Program Overview and the modality-specific program requirements that you would like to apply for before applying for accreditation. The application process for the Diagnostic Modality Accreditation Program includes six programs.

Step 2

Print out the Application Instructions and Checklist. The Application Instructions should be used as a guide as you complete the application forms and also provides definitions for clarification of frequently used terms. The instructions move you through the process of completing the application, step by step. For your convenience, we have included the Application Checklist to help ensure all items are included.

Step 3

Application forms are provided in sections corresponding to your practice site and modality-specific equipment pages. Also included is a payment worksheet to help calculate fees to be submitted with your application. You are required to submit the legal document (one per practice site location). The ACR will not process your application without the required legal form and fees.
