Testing and QC Forms
Facilities that have applied for the Ultrasound Accreditation Program will read the following documents and use the forms to gather data that will be entered into the online testing package when the images are sent for review. Please note that these forms should not be submitted to the ACR for accreditation.
IMPORTANT - The Ultrasound accreditation process has recently changed.
NOT USING THE CORRECT FORMS MAY RESULT IN A DELAY OF THE ACCREDITATION PROCESS!
If you applied for ultrasound accreditation:
- AFTER 4/27/11, use the links below.
- PRIOR to 4/27/11, please contact the ACR at 800-770-0145 and select option 6 to obtain the correct paperwork.
Facilities that have applied for the Ultrasound Accreditation Program will submit the following documents with their image submission for review.
