NRC Announces Availability of Forms and Guidance -- Training & Experience and Preceptor Attestation
RIS-2006-027 was just posted today, announcing the availability of the new Form 313A series and guidance (Training and Experience and Preceptor Attestation Forms). Click below for the link to the original RIS-2006-027 document:
PLEASE NOTE that there is an error in the Form 313A (RSO). The table in Part 1, item 3a should not list "Chemistry of byproduct material for medical use." And "Radiation Dosimetry" should be listed in this table. NRC HQ will be making corrections to the Form 313A (RSO) and the corrected form should be posted on the web site shortly. Please refer to these web sites for corrected and updated forms:
